I came across this very good prayer and I thought you might like it. The thoughts in this prayer call us to unity and faith.
ALMIGHTY God, who hast given us grace at this time with one accord to make our common supplications unto thee; and dost promise, that when two or three are gathered together in thy Name thou wilt grant their requests; Fulfil now, O Lord, the desires and petitions of thy servants, as may be most expedient for them; granting us in this world knowledge of thy truth, and in the world to come life everlasting. Amen.

Do you have the ancient source for this prayer of Chrysostom?
It was an Internet article that credited the prayer to the 1662 Book of Common Prayer just as it is referenced in the original blog.
I have no further documentation, but if I can retrieve the original notes, I will let you know.
